Job Description:

Health Care Without Walls (healthcarewithoutwalls.org) (HCWW) seeks a full-time paid Medical Director for an innovative alternative health care delivery practice of primary care for a small cohort of elderly poor/homeless women who are supported by our “Medical Home Without Walls”  model. All care is delivered onsite in homeless shelters and will be supported by ACO global payments.  We seek a candidate who will launch this initiative from the ground up and pursue NCQA certification for it.  We want a strong leader who will share our goal to effect social change in health care access through creative solutions that truly represent the patient’s needs. This position requires onsite shelter clinic sessions providing clinical care and supervision at two of Boston’s women’s shelters in addition to the administrative responsibilities of setting up and managing this project. HCWW. is a non-profit organization of physician volunteers and nurses who go into homeless shelters in Boston to provide free medical care.  Our mission aims to provide improved access to medical care for poor, homeless and battered women and their children. HCWW has been recognized locally and nationally as a viable alternative delivery model that improves access to healthcare for society’s most vulnerable citizens.  HCWW’s services fill in critical gaps in primary acute care and chronic disease management for those who do not interact comfortably with mainstream medical systems and government homeless clinics.  Our services result in enormous cost-savings in prevented ER visits, chronic disease monitoring and comprehensive case management.
